Job summary

LifeLong Medical Care seeks a Case Manager II to support patients with complex needs through outreach, assessment, and care planning. The CM II leads Enhanced Care Management and coordinates referrals to internal and external services, including housing and benefits, across home, clinic, or community settings.

Represented by SEIU-UHW, the role involves engaging with diverse populations, building strong patient relationships, and ensuring access to comprehensive health and social supports.

Qualifications

Responsibilities

• Outreach to patients at LifeLong sites and in the community to assess eligibility.
• Engage patients to build relationships and develop care plans aligned with goals.
• Facilitate referrals and assist with applications for benefits and housing.
• Maintain patient caseloads and document in required data systems.
• Coordinate care, advocate for patients, and support crisis intervention as needed.
