Job summary

WestCare Foundation seeks a Case Manager to coordinate client cases within Sanctuary programs or shelters. You will develop and implement Individual Service Plans (ISP), advocate for youth and families, and coordinate with interagency partners to ensure cohesive services.

Responsibilities include intake, case planning, crisis intervention, and community referrals, with a focus on ethical conduct, confidentiality, and strong communication.

Qualifications

• Bachelor's degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field preferred.
• High School Diploma or equivalent with two years of college is required.
• Two years of experience in providing direct counseling services to persons with alcohol and/or drug problems is preferred.
• Intake and assessment experience is preferred.
• Knowledge of the criminal justice system.
• Valid driver’s license and ability to obtain WestCare vehicle insurance is required.
• CPR Certification and First Aid Certification.

Responsibilities

• Work closely with Program Manager to implement Individual Service Plan (ISP) approach.
• Provide referral services to clients and families to community resources.
• Oversee intake and program orientation for clients and families.
• Coordinate transportation needs for clients to appointments and other destinations.
• Contribute to program development to enhance services.
• Perform administrative duties and maintain client files and reports.
