Qualifications

• Two years case management experience preferred.

• High School Diploma required.
• Alcohol and Drug Registration and actively pursuing Certification preferred.

Responsibilities

• Assess client needs and develop case management plans.
• Assist clients with transitions between levels of care.
• Coordinate communication, referrals, and linkage to services.
• Maintain client files and ensure documentation meets policy and contract requirements.
• Represent the agency at community meetings and maintain community liaison relationships.
