Job summary

WestCare Foundation, Inc. is hiring a Case Manager to ensure clients receive comprehensive medical and behavioral health services.

The role supports the Behavioral Healthcare Home model, coordinates with the treatment team, and manages HMIS data entry and documentation. The candidate will develop and implement client case management plans, coordinate resources, assist with health insurance enrollment, and participate in discharge planning to secure housing and income after treatment.

Qualifications

• Bachelor’s Degree in a social service field, preferred.
• Ability to understand substance abuse/mental health treatment concepts.
• Ability to address needs of clients with co-occurring disorders.
• Familiarity with the community’s health resources.

Responsibilities

• Enter HMIS data accurately and timely.
• Develop client case management plans with client input.
• Coordinate services with ancillary agencies and treatment team.
• Assist with health insurance enrollment for eligible uninsured clients.
• Coordinate discharge planning and secure post-discharge housing and income.
