Job summary

WestCare Foundation is seeking a dedicated Case Manager to coordinate client cases within Sanctuary programs and shelter facilities. You will develop and implement Individual Service Plans (ISP), advocate for clients, and collaborate with interagency partners to ensure cohesive service delivery.

The role requires crisis intervention availability, intake/orientation, and ongoing evaluation of ISPs, with emphasis on community resources and aftercare support as clients transition from programs.

Qualifications

• Degree preferred in addiction counseling, criminal justice, social work or psychology.
• Two years of college-level education required if no bachelor's degree.

Responsibilities

• Coordinate client cases and ISPs; advocate with interagency partners.
• Provide crisis intervention 24/7 for residents in facilities.
