Job summary

WestCare Foundation is seeking a Case Manager in Chalan Pago, GU, to support Sanctuary programs and residents in shelters. The role focuses on case management, ISP development, advocacy and interagency coordination to provide cohesive services.

Responsibilities include intake and evaluation, crisis intervention, transportation coordination, and contributing to program development. Requires related field education or two years of college, CPR/First Aid certification, and a valid driver’s license;

Qualifications

• Bachelor’s degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field preferred.
• Two years of college-level education with high school diploma or equivalent required.

Responsibilities

• Work with Program Manager to implement ISP approach and coordinate client services.
• Oversee intake, orientation, assessment and ongoing review of ISP for clients and families.
• Identify client issues and dynamics and update ISP as progress occurs.
• Provide individual and group processing, crisis intervention, and aftercare services as needed.
• Coordinate transportation and liaise with collateral agencies and court/school teams.
• Collect and report program data and contribute to program development and improvement.
