Job summary

Saint Francis Ministries is seeking a Case Manager to coordinate and deliver services for our program participants. You will participate in client assessments, conduct regular one-on-one outreach, and develop individualized Case Plans with core and supplemental curricula.

Qualifications

• Bachelor’s degree prefered in Social Work, Human Services or related field.
• Must be 21 years of age.
• Must pass organizational background and health screenings.
• Must have ability to work from home with reliable internet access.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Coordinate and deliver services to targeted program population.
• May participate in initial and ongoing assessments of clients.
• Provide weekly/biweekly participant contact to support program retention and access to services including employment support.
• Develop strategies to ensure follow-up to participants on referrals and provide sustained support.
• Use assessments to determine core and supplemental curriculum for the Case Plan.
• Provide updates to referral sources about client progress, barriers, and solutions.
• Participate in monthly supervision and adhere to SOPs, policies, and regulations.
• Work evenings and Saturdays to meet with participants.
• Document goals, progress and milestones, including employment and education plans.
• Ensure client rights are protected and report incidents through appropriate channels.

Education
