Job summary

Helio Health, Inc. is seeking a Case Manager to support recovery from substance use and mental health disorders. This is a full-time role located at the Syracuse Intensive Apartment Program.

The position offers a sign-on incentive of up to $1,500 and an hourly pay range of $21.00–$23.00. • Associate's degree required; bachelor's preferred.
• Two years of experience in behavioral health/substance abuse fields.

Responsibilities

• Maintains a case load and performs primary case work responsibilities: screening, intake, orientation; assessment, evaluation, and intervention (QHP); treatment planning (QHP); referrals; counseling; crisis intervention; patient education; case management; reporting and record keeping; coordination with other professionals; discharge planning.
• Trains staff, volunteers, and students/interns.
• Performs Quality Assurance activities; ensures services align with policies and regulations; monitors provision of services; evaluates programs; assists in problem resolution and program modifications.
• Participates in program administrative and clinical committees as assigned; represents agency to the community; other duties as assigned by supervisor.

Overview

Helio Health is looking for a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full-Time position and located at our Syracuse Intensive Apartment Program.

Pay:$21.00 - $23.00 per hour.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including:
  • Screening, intake, orientation
  • Assessment, evaluation, and intervention (Only performed by Case Managers with QHP)
  • Treatment planning (Only performed by Case Managers with QHP)
  • Referrals
  • Counseling
  • Crisis intervention
  • Patient/resident education
  • Case management
  • Reporting and record keeping
  • Consultation with other professionals
  • Coordination of care with other community providers
  • Discharge planning
• Trains of staff, volunteers, and students/interns
• Performs Quality Assurance activities
  • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitors provision of services when assigned.
  • Evaluates programs and identifies strengths/weaknesses.
  • Assists in problem resolution and implementation of supervisor approved program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents agency to the community.
• Other duties as assigned by supervisor.

Qualifications

• Associate's degree in related field required, bachelor's preferred.
• Two years of experience working with behavioral health and/or substance abuse issues.
• Candidates with lived experience encouraged to apply.
• Valid NYS Driver’s License preferred.
