Helio Health is seeking a Case Manager to support recovery from substance use and mental health disorders. This full-time role is located at our HUD Housing program, with a Monday–Friday 8:00am–4:30pm schedule.

A sign-on incentive of up to $1,500 is offered, and pay ranges from $22.00 to $24.00 per hour. Responsibilities include case management, intake, assessments, treatment planning, referrals, crisis intervention, and coordination with community providers.

Qualifications

• High School Diploma or GED required; associate's degree preferred.
• Two years of experience in behavioral health and/or substance use settings preferred.
• Candidates with lived experience encouraged to apply.

Responsabilités

• Maintain a case load and perform core case management tasks.
• Screening, intake, orientation, assessment, and intervention.
• Develop treatment planning, referrals, and crisis intervention.
• Coordinate care with community providers and discharge planning.
• Train staff and volunteers; conduct home visits as needed.
• Participate in QA, committees, and agency representation.

Overview

Helio Health is looking for a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full-Time position and located at our HUD Housing program. The schedule will be Monday- Friday 8:00am-4:30pm.

Pay:$22.00 - $24.00 per hour.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including:
  • Screening, intake, orientation
  • Assessment and intervention
  • Treatment planning
  • Referrals
  • Crisis intervention
  • Patient/resident education
  • Case management
  • Reporting and record keeping
  • Consultation with other professionals
  • Coordination of care with other community providers
  • Discharge planning
• Trains of staff, volunteers, and students/interns
• Home visits required (agency vehicles may be provided)
• Performs Quality Assurance activities
  • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitors provision of services when assigned.
  • Evaluates programs and identifies strengths/weaknesses.
  • Assists in problem resolution and implementation of supervisor approved program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents agency to the community.
• Other duties as assigned by supervisor.

Qualifications

• High School Diploma/GED required, associate's degree preferred.
• Two years of experience working with behavioral health and/or substance abuse preferred.
• Candidates with lived experience encouraged to apply.
• Valid NYS Driver’s License required.
