Job summary

Maricopa County Human Services Department is seeking a Case Manager to assess client needs, authorize services, and monitor progress to promote independence and dignity for seniors and adults with disabilities. The role involves in-home and community-based service coordination with partners across Maricopa County.

The position is hybrid in Phoenix with a contracted end date anticipated for 06/30/2027 and pay range $25.25–$33.75 hourly, depending on experience, with overtime eligibility.

Qualifications

• Two years of professional case management experience with aging or disabled populations.
• Bachelor’s degree in social work or related field.
• Ability to obtain Arizona driver’s license and DPS fingerprint clearance.
• Pass a central registry background check and pre-employment screening.

Responsibilities

• Assess, authorize, and monitor services for aging and disabled individuals.
• Provide empathetic, comprehensive, and prompt care.
• Coordinate client care with community partners and home-care providers.
• Complete case management documentation within contractual timelines.
• Utilize community resources to support clients’ well-being.
