Qualifications

- Bachelor's degree in social work or a related field required
- Master's degree in social work or related field valued and encouraged
- Moderate bilingual skills in Spanish/English are valued
Responsibilities
- Assess clients' needs and authorize services
- Monitor clients' progress to promote independence
- Coordinate care with community partners and service providers
- Complete required case management documentation within contractual timelines
