The role requires a Bachelor’s Degree in Social Services (preferred) and prior youth experience. You will engage clients, document progress, coordinate housing placements, and collaborate with local agencies to deliver wraparound services.

Qualifications

• Bachelor’s Degree in Social Services preferred.
• Prior experience working with youth is required.
• Demonstrate the ability to connect meaningfully with young people from diverse backgrounds.
• Strong communication, collaboration, and organizational skills.

Responsibilities

• Maintain a physically and emotionally safe environment.
• Meet with clients in their units, in the office, and in the community several times a month.
• Develop and maintain positive relationships with clients.
• Help secure housing by visiting apartments and ensuring cleanliness and safety.
• Complete HMIS assessments on new clients and update as needed.
• Develop, monitor, and revise client goal plans.
• Create a discharge plan with a two‑month aftercare plan.
• Maintain client files per DHS regulations.
• Document case notes and client interactions.
• Assist in moving participants into apartments.
• Teach independent living skills.
• Foster a positive team environment with open communication.
• Establish relationships with Allegheny County agencies and other providers.
• Prepare reports for funding agencies.
• Other duties as assigned.
