Job summary

SHELTER, Inc. is hiring a Case Manager for our Permanent Supportive Housing programs in Pittsburgh, CA. The role involves intake, assessment, and planning for individuals and families experiencing homelessness.

You will conduct comprehensive assessments, develop care plans, and provide financial coaching to support long-term stability. The position requires frequent travel for home visits and a 20‑person caseload, with a focus on compassionate, community-centered care.

Qualifications

• Bachelor's degree in social services or related field or equivalent experience.
• Proficiency with Microsoft Office; ability to use HMIS system for data entry and reporting.
• Experience with vulnerable populations preferred, including domestic violence survivors.

Responsibilities

• Intake and Assessment: verify income and documents; orientation on program services.
• Case Planning and Monitoring: develop data-driven care plans; drive participant-centered goals.
• Data and Documentation: keep HMIS data up-to-date and maintain case files within 24 hours of service.
• Maintain caseload of up to 20 households and coordinate with partners and participants.
