Job summary

Telecare Corporation is seeking a Case Manager II in Alameda, CA. Under supervision, you will serve as the primary case manager for individuals with mental illness, helping them live as independently as possible in their community.

Shifts are full-time, 8:30 AM to 5:00 PM, Monday to Friday. Starting pay ranges from $19.28 to $23.24 per hour, with geographic differentials and pay commensurate with experience.

Qualifications

• High School Graduation or G.E.D. equivalent with 2 years direct mental health/community services experience; or an Associates degree with 1 year; or a Bachelor's in Social Services.
• Valid and current driver's license and personal vehicle insurance in your name.
• Willingness to use your personal vehicle to drive clients to appointments, meetings and activities.

Responsibilities

• Serve as primary case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
• Provide support under direct supervision as Case Manager II.

What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available

Full-Time | AM | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday

Expected starting wage range is $19.28 - $23.24. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to attend meetings, etc.
