Job summary

Telecare Corporation in Marysville, CA is seeking a Case Manager II to provide direct case management support under supervision. You will assist individuals with mental illness to live independently in their community, coordinating services and supports through the SHINE FSP program.

Mon-Fri, 8:30 am–5:00 pm, with a starting wage range of $19.28–$23.24 per hour. GEO-based pay differentials apply, and starting pay reflects experience and location.

Qualifications

• High School Graduation or GED equivalent with 2 years direct mental health/community services experience; or an Associates degree with 1 year experience; or a Bachelor's in Social Services.
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, with weekly mileage reimbursement at the IRS rate.

What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available:

Mon - Fri 8:30 am - 5:00 pm
Expected starting wage range is $19.28 - $23.24.

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.(weekly mileage reimbursement at the IRS rate)
