Job summary

Telecare Corporation is seeking a Case Manager II to act as a primary support and case manager for individuals with mental illness and co-occurring disorders. The role focuses on helping clients live as independently as possible within their chosen community.

The position requires a high school diploma or GED, with potential for an associates or bachelor’s degree in social services. A valid driver's license and willingness to use a personal vehicle are essential.

Qualifications

• High School Graduation or GED equivalent and two years of direct services in Mental Health or community services; or an Associates degree and one year of experience; or Bachelor's degree in Social Services.
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness and other co-occurring disorders, helping them live as independently as possible in their chosen community.
• Under direct supervision, manage cases and coordinate services to support client goals.
• Assist with transportation to appointments and group activities as needed.

What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co‑occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available:

Full‑Time | AM | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday

Expected starting wage is $23.00. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to attend meetings, etc.
