Job summary

Telecare Sonoma ACT (Assertive Community Treatment) seeks a Case Manager II to provide primary support and case management for individuals with mental illness, helping them live as independently as possible in their community. The role operates on a full-time, daytime schedule (AM shift) in Santa Rosa, CA.

The position offers a stable weekday routine, a starting wage range of $23.15-$23.71 per hour, and geographic differentials based on location.

Qualifications

• High School Graduation or GED with 2 years of direct services in Mental Health or community services; or an Associate degree with 1 year of experience; or a Bachelor’s degree in Social Services.
• Must have a valid and current driver’s license with your name listed as driver.
• Willingness to use personal vehicle to drive clients to appointments, meetings, and group activities.

• Under direct supervision, coordinate services and support to help clients achieve community integration.

What You Will Do To Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available

Full-Time | AM | Shifts: 8:30 AM - 5:00 PM, Varies as needed | Days: Monday - Friday, additionally varies as needed

Expected starting wage range is $23.15 - $23.71. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring To The Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to attend meetings, etc.
