Job summary

Telecare Corporation is seeking a Case Manager II to provide primary support and case management for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.

Under supervision, you will coordinate services, arrange transportation, and collaborate with partners to connect clients with resources, programs, and ongoing support within a trauma-informed, compassionate framework.

Qualifications

• High School Graduation or GED equivalent with 2 years direct mental health/community services experience, or an Associate's with 1 year, or a Bachelor's in Social Services.
• Valid and current driver's license; personal vehicle insurance; name listed as driver.
• Willingness to use personal and company vehicles to transport clients to appointments and activities.

What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available:

Short Hour | AM | Shifts: 8:00 AM - 4:30 PM | Days: Friday - Saturday

Expected starting wage range is $23.00 - $23.71. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services


• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.


• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.


• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
