Job summary

HealthNet in Indianapolis seeks a Case Manager II to coordinate client care for housing and employment stability. You will work with households to assess needs, connect to community resources, and ensure eligible funding and documentation.

The role requires leadership in staff training and collaboration across internal and external partners. You will lead in SMART housing planning, maintain HMIS data, and support team efforts to achieve positive outcomes for clients facing barriers to housing

Qualifications

• Master’s degree in healthcare or social service field is required.
• 3–5 years of experience with vulnerable populations is preferred.
• Experience with grants is a plus.
• Strong written and oral communication skills are essential.
• Ability to work independently and as part of a team.
• Excellent organization and attention to detail are needed.

Responsibilities

• Coordinate client care to support housing stability and employment goals.
• Link clients to community resources and services.
• Maintain accurate HMIS documentation and data entry.
• Attend mandatory meetings and provide staff training when needed.
• Lead and mentor junior staff in specialty care linkages where appropriate.
• Develop SMART housing plans with clients and partners.
• Monitor progress and adjust plans to meet goals.

Education

Master’s degree in healthcare or social service field

Experience with vulnerable populations

Experience with grants (preferred)

Strong written and oral communication skills
