HealthRIGHT 360 Pasadena is seeking a Case Manager II to coordinate care across outpatient and community settings. You will guide clients through intake, assessments, benefits applications, and access to housing, healthcare, and employment resources, ensuring alignment with program goals and policies.

You will collaborate with a multidisciplinary team to support optimal treatment outcomes and provide translation services as needed.

Qualifications

• Case management experience in human services.
• Experience with rehabilitation, relapse prevention, and recovery.
• Ability to maintain HIPAA and other regulatory standards.

Responsabilités

• Complete case management assessment and enter financial and benefit information during client admission.
• Monitor client’s progress towards treatment plan goals.
• Support client in Medi-Cal application or transfer.
• Assess client’s case management needs and complete releases of information.
• Coordinate and connect clients to housing, benefits, healthcare, and employment services.
• Prepare clients for job search and aid with resume writing.
