Job summary

NorthCare is seeking a qualified Behavioral Health Case Manager II to deliver outpatient care navigation for adults, children, and families. You will coordinate integrated physical and behavioral health services, ensure care continuity, and assist with benefits enrollment, housing, and transportation resources.

Qualifications

• CM II certification and annual CE requirements required.
• ODMHSA Wellness Coach Certification and CE requirements required.
• Minimum 2 years of CM II experience in community mental health.
• Willingness to provide services in multiple community settings including home visits and telehealth.
• Proficiency in MS Word and basic computer skills.

Responsibilities

• Provide integrated services that are welcoming, engaging, accessible and community-based.
• Coordinate care across physical, behavioral health, and social needs.
• Mentor colleagues in care coordination and meet population management needs.
• Collaborate with the multi-disciplinary team to support health behavior change.
• Assist patients with enrollment in benefits, housing, transportation, and education resources.
• Maintain accurate clinical records and ensure documentation deadlines are met.
