Job summary

Qualifications

• Arizona driver's license and background clearances required.
• Ability to work independently and manage a caseload.

Responsibilities

• Drive to members' homes, schools, and program sites to discuss service needs and supports.
• Develop and monitor Person-Centered Service Plans (PCSP) and document progress.
• Collaborate with community agencies to provide assistance and support in crises.
• Write progress notes and maintain case records; prepare reports and projects.
