Job summary

The City of Norfolk seeks a Case Manager II to support adults with serious mental illness and co-occurring disorders within the Norfolk CSB. You will assess needs, develop ISP plans, link clients to services, and coordinate care with providers to promote independence.

Responsibilities include documenting progress, involving families when appropriate, and ensuring timely service delivery. Work hours are Monday–Friday, 8:30 AM–5:00 PM, with a salary range of $49,152–$56,403.

Qualifications

• Face-to-face assessment to determine functional limitations, eligibility for service, progress and need areas.
• Develops an ISP based on assessment information and reviews every 90 days.
• Links to services, resources, and supports specified in the ISP.
• Assist the individual directly by obtaining the resources needed and advocating for independence.
• Coordinates services with other providers and agencies to ensure continuity of care.
• Makes contact with family or significant others to promote ISP implementation.
• Monitors progress and consumer satisfaction with monthly service.

Responsibilities

• Faces-to-face assessment to determine needs and eligibility.
• Develops and reviews ISP every 90 days or more often if needed.
• Links to services, resources, and supports as per ISP.
• Obtains resources and advocates for consumer independence.
• Coordinates services with providers and other agencies.
• Contacts family to promote ISP and stability.
• Monitors progress and provides monthly service to caseload.
