Qualifications

• Bachelor's degree in a human services field or equivalent experience.
• Experience in case management or related human services required.
• Strong communication and coordination abilities with diverse populations.

Responsibilities

• Complete face-to-face assessments to determine needs and eligibility at least every 90 days.
• Develop and review an Individualized Service Plan (ISP) with consumer and family as appropriate.
• Link consumers to entitlements, services, and supports per ISP.
• Advocate for consumers and facilitate access to resources.
• Coordinate service planning with other providers and agencies to ensure continuity of care.
• Reach out to family or significant others to support ISP implementation.
• Provide monthly service contacts and adjust frequency as clinically indicated.
