Job summary

Telecare seeks a Case Manager II to support individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their community under supervision.

Shifts: Full-Time DAYS 8:00 AM - 4:30 PM (Sun-Thu) and Part-Time 4:00 PM - 8:30 PM (Sun-Thu). Starting wage range is $23.00–$26.00 per hour, with geographic differentials based on location. Starting pay reflects relevant experience.

Qualifications

• High School/GED with relevant mental health experience or related degree.
• Valid driver’s license and personal vehicle insurance.
• Willingness to use own vehicle to transport clients.

Responsibilities

• Serve as a primary support and case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community under supervision.
