Qualifications

• Bachelor’s degree in Nursing or related field required; master’s preferred.

Responsibilities

• Liaise with primary care and community providers to coordinate care.
• Visit clients in hospital and community settings; provide care coordination.
• Educate patients and families on health management and disease prevention.
• Develop and monitor care plans; mentor junior staff and interns.
• Promote continuity of care across services and reduce fragmentation.
