Job summary

Alameda Health System is seeking a CN II, Case Management—Care Transition & Complex Care Programs in Oakland, CA. Qualifications

• Bachelor’s degree in nursing or related field required.
• RN license in California required; BLS certification.
• One to three years acute care or case management experience preferred.

Responsibilities

• Act as liaison with primary care providers and community providers to coordinate care.
• Supervise Nursing Interns or Medical Assistants when needed.
• Communicate with physicians and health team for continuity of care and document in EHR.
