Job summary

The Alliance seeks a Complex Case Manager, Nurse in Alameda, CA, to oversee case management and care coordination for members with highly complex medical and psychiatric needs. You will collaborate with a multidisciplinary team to enhance quality of life and cost-effective outcomes.

Responsibilities include leadership in nursing processes, care planning, advocacy, and coordinating with PCPs, specialists, and community resources. Hybrid work arrangement with CA residency requirements.

Qualifications

• BSN or MSN required.
• Registered Nurse license, active and unrestricted in CA.
• Certification in case management (CCM) preferred or willing to become certified within 12 months.

Responsibilities

• Assess, plan, facilitate and advocate for members in collaboration with health resources.
• Co-manage complex cases telephonically with licensed/unlicensed staff.
• Carry a caseload of members, alone and as part of a team.
• Act as subject matter expert on nursing process for the team.
• Triage and manage all aspects of the nursing process.
