Job summary

LifeLong Medical Care (Oakland, CA) is seeking a Case Manager II to support patients with complex care needs. The CM II leads Enhanced Care Management, conducts outreach, and coordinates services across home, clinic, and community settings.

Responsibilities

• Outreach to patients meeting program eligibility at LifeLong sites and in communities.
• Collaborate with patients to develop and implement a care plan aligned to goals.
• Facilitate referrals and assist with required forms and information releases.
• Maintain caseload and use data systems to meet program and grant requirements.
