Responsibilities

• Conduct discharge planning assessments in a timely manner.
• Coordinate disposition and transfer readiness with physicians and payers.
• Educate patients and families to develop self-management plans.
• Coordinate with interdisciplinary team to transition patients to home or other settings.
• Maintain documentation meeting Medicare/Medicaid and regulatory standards.
• Provide mentoring and staff education on care coordination and advance directives.
