Job summary

East Bay Innovations is seeking a Case Manager I (Full-Time) to coordinate transitions from nursing facilities to the community, helping clients secure housing, navigate benefits, and access services. Qualifications

• Two years of experience in human services; IHSS and Medi-Cal knowledge preferred.
• Bachelor's degree, education can be substituted with additional 2 years of experience in the field.
• Valid California driver’s license, auto insurance, and access to a vehicle.

Responsibilities

• Provide coordination to help clients transition safely from institutional care to the community and create a comprehensive service plan.
• Help clients access resources such as IHSS, affordable housing, and community health services.
• Maintain accurate and detailed records including state and program documentation, case notes, billing, and files.
• Facilitate development of a support circle with family, friends and neighbors.
• Coordinate with family, other agencies, and EBI staff to support client needs.
• Communicate timely updates to the direct supervisor.
