Job summary

The HealthCare Connection, Inc. is seeking a Case Manager to support high-risk and complex patients, coordinating care and connecting them with essential community resources.

You will assess needs, develop individualized care plans, and partner with an interdisciplinary team to improve health outcomes. Responsibilities include coordinating with primary care and behavioral health teams, providing referrals for mental health and substance use treatment, and advocating for patients through

Qualifications

• Bachelor’s degree required in social work, sociology, psychology, or a related field.
• 1+ year of case management experience preferred.
• Experience with pediatric and adult populations.
• Familiarity with community resources and social services.

Responsibilities

• Identify at-risk patients using screening tools (e.g., PRAPARE).
• Conduct comprehensive assessments of patient and family needs.
• Coordinate care plans with healthcare providers and interdisciplinary teams.
• Develop and facilitate patient-centered care plans with primary care and behavioral health providers.
• Ensure effective communication to avoid duplication of services.
• Provide referrals and support for mental health and substance use treatment programs.
• Advocate for patients with problem-solving, education, and self-management coaching.
• Schedule and ensure follow-up appointments, screenings, and referrals.
• Maintain documentation and patient tracking systems.
• Build relationships with community agencies and external resources.
• Serve as a resource and liaison for patients, families, and the healthcare team.
