Job summary

Paylocity is seeking a dedicated Case Manager in Philadelphia to manage every aspect of participant care while building strong relationships with participants and their families. You will coordinate consent, care plans, Medicaid recertifications, and service changes, while supervising home health aides and CNAs to ensure timely, high-quality support.

This role involves monthly check-ins, home visits, and collaboration with the Service Coordinator to align services with individual plans.

Responsibilities

• Introductory meetings within 24 hours of person becoming a Participant/ Individual.
• Execute Participant/Individual consent form.
• Complete Direct Care Worker Assignment Sheet.
• Conduct monthly phone inquiries to discern any issues or concerns.
• Home visits at least every 90 days to adjust care plans.
• Attending Individual Support Plan meetings.
• Ensuring that recertification of Participant/Individual’ Medicaid benefits do not lapse.
• Submit consumer services survey on an annual basis.
• Serving as liaison between the Participant/Individual and Service Coordinator regarding services changes/issues. (i.e., increase of hours, temporary holds, hospitalizations, etc.)
• Ensuring timeliness and dependability of caregivers on their respective case load.
• Policy and procedure coaching and enforcement; report need for disciplinary action if necessary.
• Scheduling and conducting Meet and Greets and in service visits in the field as needed.
• Other duties as assigned.
