Qualifications

• High school diploma or GED.
• AOD Registration + 2 years of case management experience.
• AOD Certification.
• Valid CA Driver’s License.

Responsibilities

• Assess treatment needs and monitor progress.
• Coordinate care across health, substance use, mental health, and social services.
• Schedule appointments and arrange transportation.
• Discharge planning and crisis intervention.
• Maintain documentation in HIPAA and DPH compliance.
• Provide case management and care coordination for clients.
• Work with outside partners to link clients to services.
