Job summary

Behavioral Health Services at Redgate Memorial Recovery Center in Long Beach, CA seeks an experienced Case Manager II to provide comprehensive case management, recovery support, and care coordination for clients in its programs.

Under supervision, you will conduct screenings, develop care plans with clients, support housing and benefits navigation, and collaborate with the treatment team to promote ethical, evidence-based care. Strong documentation and crisis response skills are essential.

Qualifications

High school diploma or equivalent required; bachelor’s preferred for DMH program
Experience in substance use treatment or behavioral health setting is desirable
Knowledge of confidentiality requirements and DHS/HIPAA standards helpful

Responsibilities

Conduct comprehensive substance use screening, care planning, and treatment activities
Contribute to individualized care plans in collaboration with clients and treatment team
Support clients with relapse prevention, life and coping skills, discharge and aftercare planning, and recovery support
Provide case management services including benefit assistance, housing navigation, vocational support, and community reintegration.
Provide linkage and warm handoffs to community resources, including 12-Step and other recovery support groups.
Occasionally provide transportation for clients including DPSS and other linkages.
Build collaborative support systems with community employers, agencies, and housing providers.
Develop and maintain partnerships with governmental housing agencies to respond to client housing needs.
Coordinate with medical providers, social services, probation/parole, DCFS, courts, and other community partners.
Facilitate external communication for clients as approved by supervisor and as consented via Release of Information.
Prepare letters and progress reports for DCFS and other systems including required information for reporting as consented via Release of Information
May provide evidence-based services (e.g., CBT, MI, DBT, Seeking Safety) to assigned group and/or individual caseload
Maintain knowledge of evidence-based practices, medications for addiction treatment, co-occurring disorders, community resources, self-help support, and treatment philosophies
Monitor client progress and maintain accurate and timely treatment records and progress notes
May perform toxicology testing, take and record vitals as needed
May support with coaching, consultation, training, or development of new or entry-level staff
For residential cases, may make rounds and observe medication administration
Ensure compliance with regulatory and accreditation standards (DMH, DHCS, SAPC, HIPAA, 42 CFR Part 2, CARF)
Respond to behavioral crises and emergencies safely within scope
Document changes in client condition or behavior and manage emergencies
Participate in safety programming and maintain health and safety standards
Attend supervision, clinical meetings, trainings, and committees
Contribute to agency quality assurance and improvement initiatives
Maintain professional boundaries and confidentiality
