Qualifications

Responsibilities

• Document client intake and treatment progress.
• Assist clients with accessing resources and support.
• Collaborate with treatment team and external agencies.
• Coordinate housing and transportation as needed.
• Maintain accurate notes in the Electronic Health Record.
• Facilitate educational groups and classes.
