Job summary
Benchmark Human Services looks to hire a Case Manager to support individuals with intellectual and developmental disabilities and their families in Vermont. You will coordinate planning, advocacy, and services to help people live as independently as possible and participate in their communities.
The role involves traveling within the Brattleboro/Bennington region, collaborating with guardians and providers, and guiding person-centered planning to achieve meaningful outcomes for individuals
Qualifications
- Bachelor’s degree in any field + 1 year human services experience
- Associate degree + 2 years human services experience
- High school diploma + 3 years human services experience
- Valid Driver's License
- Valid Auto Insurance
- Must live in VT
- Travel up to 75%
Responsibilities
- Complete core competency training required by the State
- Perform evaluations and assessments to meet the needs of individuals served
- Support the individual in person-centered planning and assessment
- Facilitate planning meetings and scheduling
- Complete Needs Assessment Annual Review with the individual and team
- Assist in discovery of preferences, goals, and needs for planning
- Facilitate completion of budgets and planning documents
- Ensure information is available for planning
- Invite others to planning meetings
- Develop agendas and follow-up notes for planning meetings
- Support the individual to lead the planning process
- Identify modifications to rights and freedoms
- Review and update service plans and related documents
- Monitor plan implementation and wellbeing
- Respond to emergencies and incidents appropriately
- Validate that services align with the plan and outcomes
