Job summary

Benchmark Human Services is seeking Case Managers to support individuals with intellectual and developmental disabilities in Vermont. You will coordinate planning, advocacy, monitoring, and service coordination to help people live as independently as possible and participate in their communities.

The role requires collaboration with families, guardians, and the support team to develop and implement person-centered plans that align with the ISA/BIP service plan and desired outcomes.

Qualifications

• Bachelor's degree in any field with 1 year of human services experience or related education.
• Associate degree in human services with 2 years of relevant experience.
• High school diploma with 3 years of human services related experience.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals with person-centered planning goals and activities.
• Facilitate and schedule participation in person-centered planning meetings.
• Complete annual Needs Assessment Review with the individual and team.
• Assist in exploring preferences, strengths, goals, and needs for planning.
