Job summary

Benchmark Human Services is seeking a Case Manager in Vermont to support individuals with intellectual and developmental disabilities and their families within our community-based services. The role focuses on planning, advocacy, and coordinating medical, social, and educational services to enable independent living.

You will lead person-centered planning, develop and monitor service plans, and collaborate with guardians, families, and providers to ensure safety and well-being.

Qualifications

• Must have knowledge of Vermont’s public service system for developmental disabilities services and brain injury services.
• Must be able to work collaboratively while meeting individual performance goals.
• Must be able to verbally and non– verbally communicate with a broad audience of stakeholders.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist the individual to understand goals and activities of person-centered planning.
• Facilitate and schedule participation in person-centered planning and assessment.
