Job summary

Benchmark Human Services in Vermont is seeking Case Managers to support individuals with intellectual and developmental disabilities and their families. You will help coordinate planning, advocacy, and access to medical, social, and community services to enable independence and community inclusion.

In this role you will lead person-centered planning activities, monitor service plan implementation, and collaborate with guardians, family, and providers to ensure goals and outcomes are achieved.

Qualifications

• Experience supporting individuals with disabilities is recommended.
• Knowledge of Vermont's public service system is required.
• Ability to travel up to 75% and reside in Vermont.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist and support the individual in understanding goals and activities of person-centered planning.
• Facilitate and schedule participation in person-centered planning and assessments.
• Complete annual Needs Assessment Review with the individual and team.
• Support the individual in identifying preferences, strengths, goals and needs.
• Review and update person-centered service plan and related documents.
