Job summary

Benchmark Human Services in Vermont is seeking Case Managers to support individuals with intellectual and developmental disabilities in planning, coordinating, and advocating for services across home, work, and community settings.

You will lead person-centered planning, complete assessments, and collaborate with families, guardians, and providers to help individuals live as independently as possible.

Qualifications

• Valid Driver's License.
• Valid Auto Insurance.
• Must live in the state of VT.
• Travel up to 75%.
• Bachelor’s degree in any field with 1 year of human services experience or equivalent.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Support person-centered planning and related documentation with the team.
• Facilitate planning meetings and coordinate services and activities.
• Monitor implementation of the service plan and engage with guardians and families as applicable.
• Respond to emergencies and ensure actions protect health and welfare.
