Job summary

Benchmark Human Services in Vermont is hiring Case Managers to support individuals with intellectual and developmental disabilities, their families, and guardians. You will help plan, coordinate, and monitor services to promote inclusion and independence in community settings.

Applicants must be willing to work in the Brattleboro/Bennington area with up to 75% travel, possess a relevant degree or equivalent experience, and have a valid driver’s license and auto insurance.

Qualifications

• Bachelor’s degree in any field with 1 year of human services related experience.
• Associate degree with 2 years of human services related experience.
• High school diploma with 3 years of human services related experience.
• Must live in VT and be able to travel up to 75%
• Valid driver’s license and auto insurance
• Knowledge of Vermont’s public service system of developmental disabilities services and brain injury services

Responsibilities

• Perform evaluations and assessments for individuals served.
• Support person-centered planning and assessment activities.
• Facilitate planning meetings and document follow-up notes.
• Assist in developing and maintaining person-centered service plans.
• Monitor plan implementation and engage guardians, families, and providers as appropriate.
