Job summary

Benchmark Human Services is launching a Vermont program and is hiring Case Managers to support individuals with intellectual and developmental disabilities. The role focuses on planning, advocacy, monitoring, and guiding decision-making to help individuals participate fully in society.

The position involves coordinating person-centered plans, working with families and guardians, and ensuring services align with the individual’s goals. Travel is required up to 75% within Vermont.

Qualifications

• Bachelor's degree in any field plus 1 year of human services experience.
• Associate degree in human services plus 2 years of related experience.
• High school diploma or equivalent plus 3 years of related experience.
• Must live in Vermont or have residency to work in VT.
• Travel up to 75% of time.

Responsibilities

• Complete core competency training required by the State.
• Perform evaluations and assessments for individuals served.
• Assist individuals to understand goals and activities of person-centered planning.
• Facilitate and schedule participation in person-centered planning and assessment.
• Complete Needs Assessment Annual Review Protocol with individual and team.
• Assist individuals in discovery, identifying preferences, strengths, goals, and needs.
• Develop agendas and follow-up notes for planning meetings.
• Lead and provide support during the person-centered planning process.
• Identify needed modifications to rights and freedoms in planning.
• Review and update service plans and related documents with the individual.
• Monitor plan implementation, satisfaction, and well‑being through visits and guardian input.
