Job summary

HealthRIGHT 360 is seeking a Care Manager I to support CalAIM ECM initiatives within California. The role involves meeting clients wherever they are—homes, shelters, or community sites—and coordinating health and social services with an interdisciplinary team.

You will conduct intake, enrollment, and engagement activities, organize care plans, arrange transportation, schedule appointments, and document progress. Reliable transportation and a valid CA driver’s license are required.

Qualifications

• High school diploma or equivalent required.
• Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
• Valid California driver’s license and access to registered and insured transportation.

Responsibilities

• Enrollment and engagement: identify eligible individuals for enrollment and use multiple strategies for engagement (in-person meetings, mail, email, texts, telephone, community outreach).
• Client care activities: organize client care activities, share information with the care team, implement MCP activities, support treatment engagement (medication review, appointment scheduling, transportation, barriers).
• Care coordination: maintain contact with providers, ensure integrated care, refer to primary care/mental health/services, collaborate with families.
• Field-based responsibilities: meet clients at locations, conduct care management in accessible settings, maintain office access and attend trainings, submit mileage for reimbursement.
• Administrative duties: write progress notes within 24 hours, draft progress letters and court reports, participate in supervision/meetings and training.
