Job summary

National Church Residences is seeking a Case Manager in Cincinnati to deliver direct client case management for Permanent Supportive Housing residents using a Housing First approach within an integrated primary and behavioral health team.

You will perform biopsychosocial assessments, help residents access benefits, document encounters within 24 hours, and lead weekly groups with a focus on independent living skills. A relevant degree and licensure are preferred, with a driver’s license required.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• 1 year of experience in a human services or related field preferred.
• Licensed Social Worker (LSW) or Licensed Professional Clinical Counselor (LPCC) preferred; valid driver’s license.
• Proficient computer skills and ability to learn new software quickly.

Responsibilities

• Conducts bio-psychosocial assessments and develops individual strengths-based mental health care plans.
• Monitors service needs and assists clients in applying for benefits and community supports.
• Documents resident encounters in electronic records within 24 hours.
• Facilitates weekly psychosocial and educational groups and maintains minimum contact with each person on caseload once per month.
• Transports and/or accompanies clients to medical visits and other appointments as needed.
• Attends weekly Blended Management Meetings as an advocate for clients and a responsible staff member.
• Performs other duties as assigned
