Job summary

National Church Residences seeks a Case Manager I to deliver direct client case management in a Permanent Supportive Housing setting using a Housing First approach. You will conduct biosychosocial assessments and help residents access community services.

Requirements include a Bachelor's in Social Work or related field; licensure preferred; about 1 year in human services and solid computer skills are expected. A valid driver's license is also preferred.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Licensed Social Worker or Licensed Professional Clinical Counselor preferred; valid driver's license.
• 1 year of experience in human services preferred.
• Proficient in Microsoft Office and basic computer skills.

Responsibilities

• Deliver direct client case management using Housing First approach.
• Conduct bio-psychosocial assessments and develop care plans.
• Document encounters in electronic records within 24 hours.
• Transport clients to medical visits and appointments.
