Job summary

National Church Residences is seeking a Case Manager I to deliver direct client case management in Permanent Supportive Housing. You will conduct assessments, develop care plans, and coordinate benefits while maintaining documentation in electronic records within 24 hours.

The role emphasizes a Housing First approach within an Integrated Primary and Behavioral Health Team, with a focus on direct client service and collaboration across partners.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• 1 year of experience in a human services or related field preferred.
• Must have good working computer skills with basic professional software and the capacity for learning new software systems quickly.

Responsibilities

• Conducts bio‑psychosocial assessments and assists individuals and families with developing strength‑based mental health care plans.
• Monitors and follows‑up with clients regarding service needs and assists clients in applying benefits.
• Documents all resident encounters in electronic record and service log within 24 hours.
• Facilitates weekly psychosocial and educational groups; maintains minimum contact with caseload.
• Transports or accompanies clients to medical visits and other appointments as needed.
• Attends weekly Blended Management Meetings as an advocate for clients.
