Job summary

The Gathering Inn in Sacramento is seeking a Case Manager to support guests toward stability. You will conduct intake assessments, develop individualized care plans, and coordinate access to services with partners to ensure follow-through.

Responsibilities include maintaining HMIS records, coordinating referrals, leading classes, and upholding confidentiality and safety standards while engaging in ongoing staff training and professional development.

Qualifications

• A bachelor's degree with one year experience or an equivalent blend of educational qualifications and practical experience is necessary.
• Computer skills for documentation and record-keeping.
• Ability to pass a Live Scan background check.

Responsibilities

• Conduct comprehensive assessments of guests’ needs, strengths, and goals through initial intake, and develop individualized care plans in collaboration with guests
• Maintain accurate and up-to-date records of guest interactions, services provided, and outcomes, utilizing the HMIS
• Coordinate access to service providers and resources, ensuring eligibility, follow-through, and seamless delivery of support
• Work collaboratively with community partners to advocate for guests, schedule appointments, arrange transportation, and make referrals
• Facilitate and lead assigned classes or programs for guests; Uphold guest confidentiality and data privacy standards
• Address behavioral concerns calmly and respectfully, maintaining safety and harmony
• Participate in staff meetings, in-service training, and other professional development opportunities, including De-Escalation, Motivational Interviewing, CPR, and First Aid
