Job summary

SHELTER, Inc. is hiring a dedicated Case Manager I to support Contra Costa County participants and the Corporate Office. You will guide families toward housing stability, connect them with essential resources, and monitor progress to help them achieve self-sufficiency.

Responsibilities include intake, assessment, care planning, and coordinating with partners. This role requires flexibility for evenings/weekends and a strong commitment to serving vulnerable populations.

Qualifications

  • Associate's degree in social services or a related field or two-year certification; equivalent experience considered.
  • 2+ years of direct Case Management experience with knowledge of public benefits, financial resources and social services systems.
  • Experience working with low-income and/or homeless populations, including those facing substance abuse, mental health challenges, or domestic violence.
  • Crises intervention and conflict resolution skills with trauma-informed care approaches.
  • Proficient in Microsoft Office Suite and typing.

Responsibilities

  • Intake and eligibility screening, inform about services, obtain consents and verify documentation.
  • Complete HMIS Intake, VI-SPDAT, and Housing Retention assessments to gauge needs and risk.
  • Manage caseload of 15–20 shelter participants, maintain contact and coordinate with partners.
  • Develop customized care plans with participants, including children's services and school enrollment.
  • Communicate care plans to staff and set follow-up dates for interventions.
  • Connect participants with community resources to meet goals and adjust plans as needed.
  • Ensure confidentiality, consent, and mandated reporting compliance, including DV.
  • Oversee case closures with program manager, and provide post-exit housing status follow-up.

Work Location: Corporate Office and Costa Contra County

Employment Details: Full time, Non Exempt, Monday- Friday during business hours, 8am-4:30pm or 8:30pm-5pm

Compensation: $24-28/hour

About the role: We're hiring a dedicated Case Manager I for our Contra Costa County and Corporate Office Location.

As a Case Manager, you'll be crucial in supporting our shelter participants. You'll offer personalized guidance, connect them with vital resources, and empower them to achieve their goals of self-sufficiency.

What you’ll do:
Intake and Assessment
  • Conducts participant eligibility screening, informing individuals of services and obtaining necessary consents, including income and documentation verification.
  • Completes assessments (including HMIS Intake, VI-SPDAT, and Housing Retention Assessment) to evaluate participant strengths, needs, and risk indicators.
Case Planning and Monitoring
  • Carries a caseload of 15-20 shelter participants (including children) requiring housing assistance, ensuring consistent contact with program participants and other service partners.
  • Develop customized plans collaboratively with program participants, focusing on establishing goals that align with appropriate resource utilization and the required level of care and services. This includes assisting with children's services and ensuring minor children are enrolled in school and other relevant programs.
  • Communicate participant care plans to support staff and establish effective follow-up dates for interventions.
  • Connect participants with existing community resources to fulfill their care and service goals.
  • Actively adjust care plans as needed, and ensure clear communication, verbally and in writing, regarding any changes in eligibility for services and financial assistance throughout program participation.
  • Comply with all legal and procedural requirements for confidentiality, release of information, consent, and mandated reporting, including domestic violence.
  • Oversee case closures, including planning and review with participants, working with the program manager for involuntary discharges to ensure procedures are followed, and providing post-exit support with follow-up contact for housing status documentation.
Data and Documentation
  • Manage all service documentation by following established procedures for opening service files, maintaining up-to-date and accurate service data in the HMIS system within 24 hours of interaction, and ensuring data quality by reviewing discrepancies and verifying accuracy. Follow established procedure for opening service file.
What you’ll bring:
  • Associate's degree in social services or a related field, or a two-year certification in a relevant area. Equivalent experience will be considered in lieu of or in combination with formal education.
  • 2+ years of direct Case Management experience with knowledge of public benefits, financial resources and social services systems.
  • Experience working with low-income and/or homeless populations, as well as individuals experiencing substance abuse, mental health challenges, and domestic violence.
  • Skilled in crisis intervention and conflict resolution, utilizing approaches such as motivational interviewing, harm reduction, and trauma-informed care.
  • Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel) and typing skills
  • Available to work fl exible hours on an as-needed basis, including evenings and weekends, to accommodate participant availability.
  • Adept at working effectively in diverse environments with people from all socio-economic backgrounds and a wide range of personalities and professional roles, including staff, residents, local agencies, contractors, and lenders.
  • Maintains clear, firm, yet flexible boundaries, and a positive demeanor.
  • Self-motivated and accountable in managing time and resources.
  • Demonstrates professionalism and maintains high ethical standards and confidentiality. Delivers quality, organized work on time.
Position Requirements:
  • Access to reliable personal transportation required, including a DMV record that permits driver to be insured under SHELTER, Inc.’s automobile coverage.
  • Must successfully pass a criminal background check
  • Must provide TB test clearance
