Job summary

Indiana Professional Management Group, Inc is seeking a full-time Case Manager for a hybrid role serving Hamilton County, Indiana. The position combines remote work from your home office with travel to attend community-based meetings several times a week.

Applicants must reside within Hamilton County and be able to travel throughout the county and surrounding areas. A Bachelor’s degree in a human services field or related experience is required, along with a valid driver’s license and reliable

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or Bachelor’s in an unrelated field with 1 year direct experience with people with intellectual/developmental disabilities, or two years experience with aging population.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an individual support plan based on the strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field with 1 year direct experience with individuals with IDD OR two years experience with aging population
